# Creating Forms Based on a Sample

> Save forms and pages as reusable sample files (.dtf, .ptf) in ABBYY FormDesigner and create new forms or add pages based on those samples.

You can use samples to create new forms or form pages.

To create a form (or page) sample:

1. Create a form (or page) in ABBYY Form Designer
2. Select **File →** **Save As...**
3. In the **Save as type** drop-down list, select **Form Sample File** or **Page Sample File**
4. Click **Save**

By default, form samples are saved to `%USERPROFILE%\Documents\Templates`.

Form samples are binary files with the extension `*.dtf`. Page samples are saved in `*.ptf` format, which is a modified version of `*.fdd` format.

To create a new form based on a form sample, select **Form from template** in the **Form Creation Wizard**.

To add new pages to a [multi-page form](/flexi-capture/form-designer/form-multipage) based on a page sample, select **Form → Add Page →** **Based on Page Sample...**.
