Checkmark field
Checkmark fields are machine-readable form elements, usually check boxes, that respondents mark; squares, circles, and underlines are common shapes.
A checkmark field is an element of a machine-readable form (usually a square called a check box) in which a mark should be made by the person who completes the form.
A checkmark field usually has an accompanying text that explains the use of the checkmark.
Other shapes are also possible, such as polygons and ovals.
