Analysis

1. Analyze the form contents and decide on the field names and the exact wording of the explanations.
2. Choose the most appropriate form type (for example, a black and white form with raster borders, a black and white linear form, or a color dropout form) and printing method (at a printing shop, on a printer, or published on the Internet).
3. Make sure the form is machine-readable.

Design

1. Create the form on the computer.
2. Publish the form (that is, lock it for editing) so that no changes can be made to the form design. This ensures that the same source form is used to print all identical forms.
